Employees and covered spouses who did not complete the 2014 Partnership Promise requirements will be transferred to the Standard PPO effective January 1, 2015. If you receive a letter in the coming weeks stating that you are being transferred and you believe all of the requirements were met, you should submit the Appeal Form.
Detailed instructions on how to file an appeal will be included in your letter and should be followed carefully. You have until August 31, 2014, to file an appeal.
Please do not forward appeals to Human Resources. Healthways will work with members who completed a biometric screening between July 15, 2013, and July 15, 2014, but have not yet turned in a Physician Screening Form.
