If you are NOT enrolling in health care, we still need the following forms on file:  Enrollment Form (select REFUSE); Basic Term Life Insurance Beneficiary Form (beneficiary should be over age 18); Designation of Beneficiary Form (needs to be notarized); make sure you have submitted your  TCRS Retirement Form .  Scan/email these documents to ptrent@tnstate.edu or fax to 615-963-5027.  You're done!

If you are enrolling in health care coverage, it is your responsibility to review and submit all documentation to the HR department within 31 days of your date of hire.  Keep in mind, this may be different from the date on which you signed your contract. 

Watch the following videos and review the Eligibility and Enrollment Guide .  These will help you make your enrollment decisions.  If you do not enroll within 31 days of your hire date, you will have to wait until the 2018 Annual Enrollment Transfer Period (AETP) and your coverage will begin 1/1/2019.

Forms may be completed online, but they must be printed so you can sign them.  All 3 of the following forms must be completed, signed, and submitted.  Please scan/email all forms and documents to ptrent@tnstate.edu or you may fax them to 615-963-5027.

Final Notes:

  1. Your health care benefits begin on the 1st of the month following 30 days of work.  If you start on the 1st of the month (i.e. January), your health care coverage begins on the 1st of the next month (February).  If you start on the 2nd or later (i.e. January), your coverage begins on the 1st of the month after you've worked 30 days  (March).
  2. Premiums are deducted from the month-end check only.  Premiums are deducted 1 month in advance.
