Long Term Exempt (Monthly Employee) Application
The Prudential Long Term Disability Exempt Employee Plan was established by the Tennessee Board of Regents for monthly-paid or salaried employees (i.e., faculty, executive, administrative and professional employees) who are scheduled to work not less than 30-hours per week(i.e., employed to work at least 80% time or more).
The Prudential Long Term Disability Non-Exempt Employee Plan was established by the Tennessee Board of Regents for semi-monthly-paid employees (i.e., clerical, support and para-professional employees) who are scheduled to work not less than 30-hours per week (i.e., employed to work at least 80% time or more).
LTD benefits provide you with loss of income protection if you become disabled from a covered sickness, pregnancy or accidental bodily injury.
The Long Term Disability Enrollment form must be completed, signed and dated by you, and submitted to Human Resources, whether you elect to participate in the plan or decline to participate in the plan.
