In order to receive a grade for this semester's co-op work experience, all text assignments and an end-of-term report must be completed and returned to this office one week before the final exam week. . Students enrolled in co-op classes during the first summer session must complete reports one week before end of session. Final Employer Evaluation and Student Self Evaluation forms can be down loaded from the Co-op office website: http://www.tnstate.edu/oce. Please return these forms together with the above stated material. As with your text activities, evaluation forms and report can be faxed to this office (615)963-7467. The following areas of inquiry should be addressed in the eight page end-of-term report:
1. Give a general description of your actual job assignment.
2. Describe your work assignment as it pertains to you department's objectives.
3. Give examples of methods used to perform work assignments and obstacles, if any, overcome in the process.
4. Which parts of your academic training prepared you for your co-op work assignment and what classes will you need for your next work assignment?
5. How has your co-op work assignment affected your preparation for your intended career path? Explain in detail.
