Most of the forms listed on this page are available in two formats: Microsoft
Word Version
Available
as a downloadable form which can be completed and printed while online. When
the form comes onscreen, enter the appropriate information in the shaded
fields
provided. You can advance from one field to the next by pushing the
"Tab" key on your keyboard, or by clicking in a field with the
mouse. You are still required to sign each printed form and submit it
to the appropriate person.
Acrobat
Version
Available
only as a viewable and printable document - it cannot be edited. Adobe
Acrobat Reader, available as a free download, must be installed on your
computer. To download Acrobat Reader, click the "Get Acrobat
Reader" icon below. Follow the onscreen instructions. The Acrobat Reader download site
and the TSU Web Server are extremely busy during some periods of the
day. Downloads can be excruciatingly slow and/or you may get any of several
error messages. The best time to
attempt an Acrobat Reader download is before 9:00 am CST.
