Concerns or Complaints that Fall Outside the Realm of Due Process

The program is committed to ensuring that unusual occurrences including complaints that fall outside the realm of due process are addressed expeditiously. Individuals or parties who are not enrolled or employed by the institution (such as clinical instructors, parents, health or professional facilities, alumni, volunteers, employers, or any other member of the public) can register their concern or complaint using the Unusual Occurrence and Complaint Form. Information required to complete the form includes:

  • Source of complaint or type of unusual occurrence
  • Contact information of the complainant
  • Description of the complaint or unusual occurrence

The form can be emailed to the Department Chair for review and disposition. If the complaint involves the Department Chair, then the form can be sent to the Office of the Dean of the College of Health Sciences. Information regarding the complaint will be treated as confidential to the extent allowed by law. Complaints are kept on file by the Department Chair or the Dean of the College of Health Sciences (if the complaint involves the Department Chair).

Individuals or parties wanting to file a complaint using the procedures outlined above shall be entitled to do so without fear of retaliation, interference, coercion, or discrimination.
